look at the screensaver settings... is it set to turn off the monitor after a certain timeframe or is the sleep set to 5 hours??? due to sleep and hibernation problems in the past I just completely turn those off and set it to turn the monitor off after 20 minutes but to never turn off hard drives or anything liek that.

I'm just throwing this out there, and I'd lean towards what screwball said being the most probable cause, but did you try just re-seating the card? I know that sounds trivial and simple, but you never know. Your system has been apart and together again a few times recently though, so I doubt if the cleanliness of the PCI-E contacts is an issue. Just a thought.
